RELEASE AND PERMISSION
I, the undersigned, am a parent of
____________________________, who is participating in the Pierce Soccer
Association Tournament. I give my permission and consent for him to
participate in the tournament. I agree to release, indemnify and hold
harmless Pierce Soccer Association, officials, sponsors, coaches, referees,
and/or other representatives from any claims arising out of any injury to my
child.
I furthermore believe that my child is physically fit to play soccer and does not have any disabilities, handicaps, present injuries or other medical condition that would prevent my child from playing.
